PROJECT DESCRIPTION

  • Reducing Diabetes Hospital Admissions

Picot: In patients distressed with Type 2 Diabetes mellitus (T2DM), does adherence to a lifestyle change counseling program, contrasted with patients who did not stick to the program, lead to a decrease in admission to the practicum site in 8 weeks or less?

According to the Centers for Disease Control (CDC), the age-adjusted prevalence of diabetes and hospitalization rates increased significantly between 1999 and 2016 because of an increase in contributing factors such as obesity, sedentary lifestyles, stress, and lack of awareness about diabetes and its consequences (CDC, 2020).

Electronic counseling sessions will be directed by practicum counselors to raise awareness among diabetes patients and help them set lifestyle goals (such as reducing sugar and carb content, working out for 6 hours consistently, and others). PROJECT DESIGN AND METHODS

Target Population Undergoing the Practice Change: Projects including powerless populations are prohibited. Powerless populations include youngsters, prisoners, pregnant ladies, intellectually disabled persons, or fiscally or educationally disadvantaged persons. The target population will be patients tormented with Type 2 Diabetes Mellitus and the nursing and subordinate support staff at the practicum site.

Inclusion Criteria: (Describe the criteria for qualified participants.) All full-time, part-time, and contract RNs and subordinate staff on the clinical surgical unit. Nursing assistant staff consolidates nursing aides, and nursing leadership includes charge nurses, nurse managers, and departmental nursing supervisors.
